Secondary hyperparathyroidism

Symptoms

Symptoms generally relate to the underlying cause of secondary hyperparathyroidsim.

Rickets in children may cause weakness, poor growth, bowed limbs, swollen joints, bone pain, fractures, or delayed tooth development.

Patients with kidney failure may have osteomalcia, osteoporosis, or both. Bone pain or fractures may occur.

Symptoms of malabsorption (such as diarrhea) or of an underlying cancer may occur in patients with those disorders.

Signs and Tests

Signs may include bone deformities, swollen joints, or fractures.

Tests may show:

  • elevated intact PTH
  • low serum calcium
  • elevated serum phosphorus
  • elevated serum alkaline phosphatase
  • variable Vitamin D ( may be normal in rickets, low in kidney failue; total Vitamin D may be low in rickets, normal in kidney failure)
  • X-rays may show evidence of osteomalacia, fractures, or subperiosteal bone resorption
  • bone mineral density may be low (osteomalacia) or elevated (chronic kidney failure)
  • urine calcium may be low
