Prostatitis - non-bacterial

Overview | Symptoms | Treatment | Prevention

Alternative names:

NBP; Prostatodynia; Pelvic pain syndrome

Symptoms

The symptoms of non-bacterial prostatitis are identical to chronic prostatitis. The difference between the two diseases is the ability to determine the cause of the prostatic inflammation. The common symptoms associated with non-bacterial prostatitis include:

Signs and Tests

A physical examination is usually normal except some patients may have a prostate that is swollen, firm, warm, and tender.

Triple-void urine specimens may be collected for urinalysis and urine culture:

  • #1 initial stream
  • #2 mid-stream
  • #3 after prostatic massage by examiner

A urinalysis reveals increased white blood cells without bacterial growth upon culture of urine #3. Blood may be detected in the urine. Prostatic secretions contain increased levels of white blood cells and also don't have bacteria.

A semen culture may also be requested. The semen culture will also not demonstrate bacterial infection. Semen analysis of patients with non-bacterial prostatitis may demonstrate increased numbers of white blood cells decreased number of sperm with poor mobility.
